mission of the Family Planning program is to provide individuals the information and means to exercise personal choice in determining the number and spacing of their children. This program offers a
broad range of acceptable and effective family planning methods, including
natural family planning methods, infertility services, and
services to adolescents.

1:00 p.m. to 4:30.
LATE CLINICS:
| By Appointment Only. 1st & 3rd Wednesdays each month from 10:30 a.m. - 7 p.m. Closed 1-2 p.m. for lunch during late hours. Beginning in November 2007 Lunch will be from 2-3pm |

Eligibility and Cost
Parental consent not necessary for
adolescents.
Women of childbearing age and men are eligible.
A sliding fee scale is used to determine a client's ability to pay.
Federal poverty guidelines are used as standards in
determining costs.
Medicaid is accepted. Plan First Insurance Applications are available at
our Clinic to cover Family Planning costs.
